Pereira Abstract After being staggered by numerous negative trials in 2013, the interventional stroke community saw four years of vindication for mechanical thrombectomy showing efficacy and safety, even beyond a hopeful 6-8 hour window out to 24 h.… Objective: To review recent endovascular intervention trials for acute ischemic stroke.

Endovascular therapy has been shown to significantly improve outcomes and reduce long-term disability after ischemic stroke. These procedures must be performed as soon as possible: Medications delivered directly to the brain. Acute ischemic stroke patients who are eligible for intravenous (IV) tissue plasminogen activator (tPA) should receive IV tPA, regardless of whether mechanical thrombectomy is being considered.

BackgroundSeveral trials involving patients with acute ischemic stroke have shown better functional outcomes with endovascular treatment than with conventional treatment at 90 days after initiation

The role of endovascular therapy in the treatment of acute ischemic stroke (AIS) must be carefully considered, given the remarkable advancement that imaging and endovascular treatment modalities have undergone over the past several years.

There have now been 12 randomized controlled trials (RCTs) on endovascular stroke therapy (EST), with eight of the last nine showing positive results – stunningly positive. Currently, systematic intravenous alteplase administered within 4.5hrs after symptom onset is the mainstay of therapy, however many question its risk/benefit ratio in ischemic stroke. 4.5 hours is a narrow therapeutic time window and many contraindications such as recent surgery, coagulation abnormalities, and history of intracranial hemorrhage inhibit many patients from receiving systemic thrombolysis. Doctors sometimes treat ischemic strokes directly inside the blocked blood vessel.

However Endovascular treatment of stroke is the non-surgical treatment for the sudden loss of brain function due to blood clots. The treatment uses microcatheters (thin tubes visible under X-rays) which are inserted into the blood clot from the groin or the arm. The blood clot is removed from the blood vessel – this procedure is called a thrombectomy.
